Pragmatic Free Trial Meta
Pragmatic Free Trial Meta is a non-commercial open data platform and infrastructure that supports research on pragmatic trials. It gathers and distributes clean trial data, ratings and evaluations using PRECIS-2. This allows for diverse meta-epidemiological analyses to examine the effect of treatment across trials of different levels of pragmatism.
Background
Pragmatic trials are becoming more widely recognized as providing real-world evidence for clinical decision making. The term "pragmatic" however, is not used in a consistent manner and its definition and evaluation require further clarification. Pragmatic trials are intended to inform clinical practices and policy choices, rather than prove a physiological or clinical hypothesis. A pragmatic study should strive to be as close as possible to actual clinical practices, including recruiting participants, setting up, implementation and delivery of interventions, determining and analysis results, as well as primary analysis. This is a major distinction between explanatory trials, as defined by Schwartz and Lellouch1, which are designed to test the hypothesis in a more thorough way.

Additionally these trials should strive to make their results as applicable to current clinical practice as is possible. This can be achieved by ensuring that their analysis is based on the intention to treat approach (as described within CONSORT extensions).
Despite these criteria, a number of RCTs with features that defy the notion of pragmatism were incorrectly labeled pragmatic and published in journals of all kinds. This can result in misleading claims of pragmaticity and the use of the term needs to be standardized. The development of the PRECIS-2 tool, which offers a standard objective assessment of practical features is a good initial step.
Methods
In a practical trial the goal is to inform policy or clinical decisions by showing how an intervention could be integrated into everyday routine care. This differs from explanation trials, which test hypotheses about the cause-effect connection in idealized conditions. In this way, pragmatic trials can have a lower internal validity than studies that explain and be more susceptible to biases in their design analysis, conduct, and design. Despite their limitations, pragmatic studies can provide valuable information to make decisions in the context of healthcare.
The PRECIS-2 tool scores an RCT on 9 domains, ranging between 1 and 5 (very pragmatist). In this study the areas of recruitment, organization, flexibility in delivery, flexible adherence and follow-up received high scores. However, the main outcome and the method of missing data was scored below the pragmatic limit. This suggests that it is possible to design a trial that has excellent pragmatic features without harming the quality of the outcomes.
However, it is difficult to determine how pragmatic a particular trial is since the pragmatism score is not a binary characteristic; certain aspects of a study can be more pragmatic than others. Furthermore, logistical or protocol modifications during the course of a trial can change its score in pragmatism. Koppenaal and colleagues discovered that 36% of 89 pragmatic studies were placebo-controlled or conducted prior to licensing. They also found that the majority were single-center. They aren't in line with the usual practice and are only called pragmatic if their sponsors accept that such trials are not blinded.
A common aspect of pragmatic studies is that researchers try to make their findings more meaningful by analyzing subgroups of the trial sample. This is due to the fact that adverse events are typically self-reported, and therefore are prone to delays, inaccuracies or coding differences. It is essential to increase the accuracy and quality of the outcomes in these trials.
Results
Although the definition of pragmatism may not require that all trials be 100 percent pragmatic, there are some advantages of including pragmatic elements in clinical trials. These include:
By including routine patients, the trial results can be more quickly translated into clinical practice. However, pragmatic trials have their disadvantages. For instance, the right kind of heterogeneity can allow a study to generalize its results to different patients and settings; however the wrong type of heterogeneity may reduce the assay's sensitivity, and thus lessen the ability of a trial to detect even minor effects of treatment.

Numerous studies have attempted to categorize pragmatic trials with various definitions and scoring systems. Schwartz and Lellouch1 developed a framework for distinguishing between research studies that prove a clinical or physiological hypothesis and pragmatic trials that inform the selection of appropriate treatments in the real-world clinical setting. The framework was composed of nine domains assessed on a scale of 1-5 which indicated that 1 was more informative and 5 being more pragmatic. The domains included recruitment and setting up, the delivery of intervention, flexible compliance and primary analysis.
The original PRECIS tool3 was an adapted version of the PRECIS tool3 that was based on the same scale and domains. Koppenaal et al10 developed an adaptation of the assessment, dubbed the Pragmascope, that was easier to use for systematic reviews. They discovered that pragmatic systematic reviews had a higher average scores in the majority of domains, but lower scores in the primary analysis domain.
This distinction in the primary analysis domains could be explained by the way that most pragmatic trials analyze data. Certain explanatory trials however do not. The overall score was lower for pragmatic systematic reviews when the domains on the organization, flexibility of delivery and follow-up were merged.
It is crucial to keep in mind that a study that is pragmatic does not necessarily mean a low-quality study. In fact, there is a growing number of clinical trials which use the word 'pragmatic,' either in their title or abstract (as defined by MEDLINE but which is neither precise nor sensitive). These terms may indicate a greater appreciation of pragmatism in abstracts and titles, but it's unclear if this is reflected in content.
Conclusions
As appreciation for the value of real-world evidence grows widespread, pragmatic trials have gained momentum in research. They are randomized studies that compare real-world alternatives to clinical trials in development. They are conducted with populations of patients that are more similar to those who receive treatment in regular medical care. This method has the potential to overcome limitations of observational studies, such as the biases associated with reliance on volunteers and limited availability and the variability of coding in national registry systems.
Pragmatic trials also have advantages, like the ability to use existing data sources and a greater likelihood of detecting meaningful differences from traditional trials. However, they may have some limitations that limit their effectiveness and generalizability. The participation rates in certain trials may be lower than expected because of the healthy-volunteering effect, financial incentives or competition from other research studies. Practical trials are often restricted by the need to recruit participants on time. Some pragmatic trials also lack controls to ensure that the observed differences aren't caused by biases during the trial.
The authors of the Pragmatic Free Trial Meta identified RCTs published from 2022 to 2022 that self-described as pragmatism. They assessed pragmatism using the PRECIS-2 tool, which includes the domains eligibility criteria and recruitment criteria, as well as flexibility in adherence to interventions, and follow-up. They found that 14 trials scored highly pragmatic or pragmatic (i.e. scoring 5 or higher) in at least one of these domains.
Trials with a high pragmatism rating tend to have broader eligibility criteria than traditional RCTs that have specific criteria that are not likely to be found in the clinical setting, and include populations from a wide range of hospitals. These characteristics, according to the authors, could make pragmatic trials more useful and useful in the daily practice. However, they don't guarantee that a trial is free of bias.
